Reaching Last-Mile Communities: CHIN Launches Community Care Connection in Rural Kogi State

Lagos, Nigeria — Citizens Health Initiative Nigeria (CHIN) has launched its Community Care Connection program in rural Kogi State, taking health outreach services directly to communities that have long been left behind by Nigeria's health system.

Delivered in partnership with local health authorities, the program is reaching over 15,000 people across hard-to-reach areas of Kogi State. Outreach services include primary care, routine vaccinations and community health education, with a strong focus on maternal and child health.

"These are communities where the health system has never shown up," said a CHIN spokesperson. "Community Care Connection is our commitment to change that — by going to the people, not waiting for the people to come to us."

The program directly addresses the geographic barriers that keep rural Nigerians from accessing basic care. Through sustained community outreach and the deployment of trained health navigators, CHIN is working to dismantle those barriers systematically across Kogi State.

Maternal and child health is a central priority, targeting preventable deaths from pregnancy and childbirth complications and childhood illnesses that remain largely vaccine-preventable.

CHIN invites individuals, organisations and government partners to support the expansion of Community Care Connection to additional states.
